Insurance Reimbursement Guide
For Clients of Adept PhysiologyStep 1: Verify Your Out-of-Network Coverage
Contact your insurance provider and ask the following:
Do I have out-of-network benefits?Are services by an ACSM-Certified Exercise Physiologist eligible for reimbursement?Is a physician clearance or diagnosis required?What documentation is needed to submit a claim?Step 2: Complete the Physician Clearance Packet
Before beginning your service, your physician must complete the Adept Physiology Physician Clearance Packet.
This form may include ICD-10 diagnosis codes that support your claim. These will be reflected on your superbill.Step 3: Gather Your Documentation
To submit your insurance claim, prepare the following:
- Superbill (provided by Adept Physiology)- Proof of payment (receipt or invoice)- ICD-10 code(s) (from your physician clearance)- CPT codes (included in your superbill)- (Optional) Cover letter explaining the nature of the service DOWNLOADStep 4: Submit Your Claim
Send your complete packet to your insurer via their preferred method:
Insurance portal
Fax
Mail
Processing times typically range from 2–6 weeks.Step 5: Follow Up
If you haven’t received a response, follow up with your insurer to confirm receipt and check the claim status.
Keep copies of everything you submit.Important Notes
